Provenance

Retrieved facts and chunks carry identifiers and source metadata. Answers return citation records, and a citation can be resolved into the permitted source excerpt and provenance detail.

Provenance lets a reader verify whether evidence actually supports the answer, discover stale context, and identify the source that should be corrected when knowledge is wrong.

A citation proves which evidence supported a result; it does not prove the source itself was correct. Source quality and conflict review still require human judgment.
